2004 in operation of the motor, shaft extension end bearing temperature rise sharply, oil bearing cover color, accompanied by anxious burnt flavour of smoke.
Operation personnel on duty after downtime inspection found that the motor shaft extension end bearing burned down, and the rotor, holding in death of the motor.
1 accident investigation after the accident, the maintenance staff to the motor strip inspection, found that: (
1)
Burning motor shaft extension end bearing, bearing frame, roller badly worn, fall off, bearing outer ring local blue;
(
2)
Bearing chamber has obvious wear trace, accompanied by a deep crack;
(
3)
Rotor shaft and the inner and outer oil cover junction, how deep grinding crack;
(
4)
Motor shaft extension end bearing inner and outer cover burn oil.
Collapse after check, and check the motor maintenance records, found that ever change on both ends of bearing in motor, two years ago.
Found in replacing bearings, motor shaft extension end bearing and the bearing chamber tolerance matching problem, bearing a slight cylindrical phenomenon, but not for processing.
After replacement of bearing, the motor only in February 2004 supplementary come on time, in addition, no maintenance record.
Root cause analysis (2
1)
Maintenance staff go on bearing cylindrical defects, lead to fever, burned after bearing in the long run.

Maintenance staff failed to timely to refuel motor maintenance.
According to the requirements of codes, the motor added refueling cycle to 2 000 h;
Clean bearing, replace grease for a period of 1 year.
But in more than two years running, adding only come on time, and not replacing the grease.
Because the motor running environment is poorer, combined with the long-term running, make oil deteriorated, bearing the lack of oil, bearing caused by burn.

When the motor bearing oiling, squarely in the winter, the temperature is low, oil increased hardness.
Due to improper operation method of gas, oil is not fully enter the bearing internal, not good lubrication, bearing caused by burn.

Operators after the fever phenomenon was found in the motor, not with in situ accident emergency shutdown button failure equipment in time, but the first report to the leadership, and then shut down the equipment, prolong the processing time of the accident, cause an accident.
Three preventive measures and countermeasures,
1)
Replace the motor bearings, bearing housing and shaft extension end cover, inside and outside oil and repair of wear and tear of the motor rotor axis.

Set the motor bearing oiling maintenance regulations, clarify and emphasize again added refueling and overhaul period of motor bearing.
(
3)
Organize maintenance personnel further learning motor maintenance procedures, improve the business skills.
Strengthening the management of equipment maintenance organization.
Requirements at the same time, in winter to run equipment added refueling, should be appropriate for motor bearing oil heating cover, to make the new grease can be fully into the bearing.
